Liver transplant: Many of the patients with an end-stage alcoholic liver disease get an orthotopic liver transplants on daily basis. Without transplant, five-year is a maximum survival rate. It is a waiting times until donor organs become available, but sooner you start a process the better it is. Consider transplantation as a potential option and call a transplant center for evaluation asap. Good luck.

Liver Failure: Sorry to say, but the only definitive therapy is a liver transplant. A low animal protein diet along with certain medications may help with mental confusion. Fluid restriction and diuretics may help with excessive abdominal distention. Any fever worsening confusion abdominal pain, black stool or vomiting of blood should be considered a life threatening emergency.

Stop drinking: First, he needs to stop drinking if he hasn't already. Second, he needs to take the medications his liver or primary care doctor has prescribed for his liver. Third, if he stops drinking and undergoes an extensive evaluation (he may have already had this evaluation), he could be considered for a liver transplant, but I do not know enough about his case to say whether that is a possibility.
