Yes: Is this is in mcg/dl is very high ( normal levels : 4.5 to 12.5 total T4 ). If, however, the units are nmol/l then it is normal. He might in a thyroid storm and needs to be check out and treted asap.

Thyroid disease : T4 level assays can range from 60 -150 for normals. Propranolol is used to effectively relieve the symptoms of palpitations, tremor, and anxiety. It has no effect on thyroid hormone secretion. Tsh is and excellent test to see if thyroid disease is under adequate control.
