My take is ...: I assumed you meant severe persistent voiding symptoms with no or poor response to all conservative Rx including antibiotics. If so, it's time to refine how to deduce more accurate Dx ... to rule out interstitial cystitis(IC) / chronic pelvic painful symdrome (CPPS) besides UTI.
