How to get answer: You seem to have most of the underlying causes answered, such as low levels of vitamins D and B-12, and this strongly points toward the reason for the pain. Although asymmetry is unusual, you almost certainly have a small fiber neuropathy, which could be proven by skin punch biopsy. You are already initiating Rx, but I would add a medical food MetanX. This will take months to reverse.
