Acid reflux: Acid reflux can cause back pain especially if there is inflammation in the esophagus. This kind of acid reflux really deserves seeing a doctor about to evaluate for further testing and treatment.

NO: Anxiety associated with pain due to acid-reflex brings upper back tension. Anxiety purely can manifest as both symptoms. Usually acid reflex does not casue back/upper back problems.
