Low: Or normal ESR suggest no infection, or cancer or inflammation. Be evaluated for neck as source of pain.

Pain in backradiatin: Low ESR is okay. That means you do not have any inflammatory process going on. Pain in the back with radiation to the arms could be some pain arising from pinched nerve radiating pain. You should see a dr. Get some examination and work up. If you have associated headache get your BP checked and see dr asap.
