Periodontitis: Bone loss from periodontitis can alter the bony support to such an extent that the tooth literally falls out of your mouth. By that stage, the rest of your teeth are probably not too far behind. Dentures may be next.

Gum disease: Normally, severe periodontitis (gum disease) is the biggest contributor of teeth "just falling out." severe decay may have also undermined the tooth/root structure, and even though you may think that all of your tooth may have fallen out, it might just be the crown, with the root still embedded in the tissue and bone.
