Yes: If serious and untreated can even lead to death, patients with suspected pneumonia need medical attention right away.

Depends: There were certainly patients in the 19th and 20th centuries that had no access to treatment and or antibiotics when appropriate that survived. However, many died and some had scars on their lungs that continued through life.Much also depends on the type of germ involved. Many pneumonias are viral and are no better with today's antibiotics than they were without.
