It varies: As a pcos expert, i can say there's a spectrum along which women with pcos lie. It can vary from mild to severe. Some "cysters" conceive quite easily while others struggle and need help. It's imperative to assure that one truly has pcos first, as several hormone imbalances can mimic pcos. This way, if treatment is needed, it can be optimized to assure the highest odds of success!

Not much: Long term follow up of pcos patients indicates that they spontaneously conceive and give birth at a very high rate similar to women without pcos. About 75% ultimately deliver at least one baby without help. Miscarriage is not significantly increased in women with pcos.
