Many things: An elisa test for HIV is very sensitive which basically means it is a good test for ruling out HIV and is only reliable when negative. A positive elisa test does not mean someone has HIV and is frequently called a "false positive" there are many causes of a false positive which include liver disease, blood tranfusions, infections, autoimmune disease and dumb luck!
