Bone loss: If you are actually seeing a hole in the bone on x-ray, that's different. I have seen cysts in the patella arising from the fracture line, making a circular hole. Some of the bone fragments don't heal, and resorb, causing a bony defect. It is not unusual to develop arthritis after a significant patella fracture.
