Can heart inflammation occur with rheumatic fever and not leave permanent damage?

Yes: If rheumatic fever causes damage, it typically effects the heart valves. It sometimes does not show damage for several years after the rheumatic fever has passed. However, depending on the measurement of heart inflammation, evidence can be present, but not become a problem with the hearts function, future infection, or even permanent signs of damage.
