Yes they can: Yes doctors do not have to treat everyone. Sometimes it a business decision. They don't want to accept a low reimbursement rate or an insurance company that is difficult to deal with.

Dental? Medicare: Except when associated with a medically covered procedure, dental care is not covered under medicare. See: http://cms.Hhs.Gov/medicare/coverage/medicaredentalcoverage/index.Html to my knowledge, dentists do not have to accept patients nor accept coverage, as long as they do not discriminate (by sex, religion, etc) or under contractual agreement.
