Depends...: ...On the location. Simple athlete's foot & jock itch will often clear with any of the over-the-counter antifungal creams or sprays. More extensive skin involvement usually requires the addition of an oral antifungal (ketoconazole, terbinafine, etc.) if it's on the face, in the scalp, or in toenails or fingernails, topical treatments simply don't work, and oral antifungals are the only way to go.
