Blocked gland: This is a blocked skin gland. The secretion is a clear mixture of oil and water. If blocked the water is absorbed, leaving a hard lump of white oil. This can be ignored or it can be removed in a simple office procedure with a sterile needle to open the top and cotton tip applicators to squeeze out the oil lump.
