Arrhythmia: If you previously had an arrhythmia and your Doctor prescribed medications to control the heart rate , the arrhythmia could be present despite normal heart rate range. There are other arrhythmias in which heart rate can be between 60-100 bpm.

It depends: It depends on the type of test which he has had done. Most commonly such reports come from a routine chest x-ray which is not always reliable for mild enlargment. I would not worry about it unless he has any reason that could cause enlargemnent of heart such as high BP or heart murmur. If there is any ongoing concern a cardiologist can check him further with an echocardiogram which is more reli.

There is a: documented increase in heart disease risk in RA patients, both from obstructive coronary artery disease as well as other vascular or heart muscle inflammation. It is only one of several health threats for RA patients. Others include infection due to immunity alterations, increased cancer risk, etc. There is often some reduction in life expectancy but the amount depends on disease severity/duration
