If your diabetic: Don't touch it go see a podiatrist. If your not diabetic and otherwise healthy, with a sterile instrument gently pierce the roof and allow fluid to ooze out. Cover with bandaid.

With a sterile: Instrument i would pierce the blister just through the roof to allow the contents to ooze out. I would not remove the roof. I would cleanse the skin first. I would not attempt this if you are diabetic or were immunocompromised without a foot doctors involvement.
