Ischemia: Ischemia is a technical term for "lack of oxygen". Microvascular means the smallest of blood vessels. Think of the plumbing in your house. The microvasculature would be your shower head.
When there is a malfunction of the microvasculature either because of spasm or buildup because of cholesterol, smoking, diabetes etc.. The organ with the bloodvessels ( ex. Heart) will malfunction.
