When I do a: comprehensive assessment I ask why you came as well as your medications, drug use, caffeine, tobacco & supplement use. Past & family psychiatric history. Safety evaluation. Childhood history. Medical and past surgical history. Information about nutrition & spirituality. Pain. Ask about symptoms of depression, anxiety, PTSD, psychosis etc.

A good interview : A mental health assessment involves an interview by a mental health expert to help identify your problem. It usually starts with open ended questions and then more specific questions are asked to narrow possible reasons for your problem. Sometimes questionnaires are used to identify problems like depression or adhd. Remember the assessment is to help you so don't be afraid to ask questions.

Each therapist has to conduct interviews in a manner that fits best with their own style. Often for me it seems to relax people by talking about less intense problems first. I will ask less emotional data specific questions, a process I call "digitilization". Once they are more comfortable talking then the emotionally laden questions are likely to be answered more honestly and in neccesary detail
