It depends: Microangiopathy (or microvascular disease, or small vessel disease) is an angiopathy (i.e.Disease of blood vessels) affecting small blood vessels in the body. It can be contrasted to macroangiopathy ( large vessel disease). Cerebral ( brain) and coronary (cardiac) small vessel disease are the examples. It depends how severe it is to cause problems. They are usually chronic in nature.

Vascular/hematologic: Depends on the source of the microangiopathy. If it is age/atherosclerosis there are progression risk reduction strategies such as blood pressure, lipid control. If you have a microangiopathic hemolysis, that requires a specialist and is an urgent matter that requires immediate evaluation and management.
