Back Pain: All 3 issues you described may cause back pain. Certainly i would suggest you start with your primary care doc. I am sure there will be some initial testing. There may be subsequent referral to pt, pain doc or spine doc. Hope that helps.

You may need 2: A general surgeon or breast surgeon would be the best as they can evaluate you for a possible breast reduction. Insurance companies often need proof of back pain associated with the large breasts in order for any reduction surgery to take place which either your pcp or an ortho could provide you after an exam.
