Fungal infection.: You might need a second course of antifungal cream. If you are sexually active, i suggest you ask also your partner if she has also fungal infection so both of you can be treated at the same time. Avoid too much soap as it decreases the skin immunity and increases the risk of fungal infection.

May have come back: The tineas cruris (fungal jock itch infection) may have never fully resoled or it came back. Other than that, it's a different dermatitis that you should get evaluated. Best wishes.
