I have a visible "lesion" on cervix during pap smear, what is leading to this?

Needs further eval: I assume this means that the doctor doing the pap smear saw a lesion on the cervix during the pelvic exam. Such a lesion could be a cancer, a benign polyp, a wart, a prolapsed fibroid, or another condition. The pap smear may give some clues to the diagnosis, but most often another special exam called a colposcopy with a specific biopsy of the lesion will be needed to make sure it is not cancer.
