Echo is best test: An atrial septal defect, especially a small one, is likely to be missed on a ct of the chest. The best way to identify an ASD is with an echocardiogram. If an ASD is found, more sophisticated tests, such as cardiovascular MRI and radionuclide imaging, can further define the size and risks of an asd.

Usually not: But it can show some indirect evidence. You might want to get 2 d echo or trans-oesophgeal echo is you have a suspicion of asd.
