See answer: A true kidney infection, aka acute pyelonephritis, is a more severe type of UTI and typically treated with 10-14 days of antibiotics based on results of a urine culture. Persistent pain raises the question of possible incorrect diagnosis, inadequate treatment, or a complicating factor (e.g. Abscess, obstruction, stone). Suggest follow-up u/a, urine culture, possible imaging study, and md visit.
