Both: Medications and psychotherapy together are most effective. Both have a role and work together. Depending on your severity, you should work on your substance abuse first, then address your mood issues if you have to choose only 1 professional to start with.

Both: Although some psychiatrists are blessed to do both medication managment and psychotherapy - most in the day of managed health care are not. Are you involved in narcotics anonymous ? An excellent, free, confidential program? That is a great place to start. Seeing a psychologist for therapy and a psychiatrist for psychiatric medication managment are also options. Take care.

Psychotherapy is: More often done by a clinical psychologist these days. A psychiatrist can help you with medication for bipolar if indicated. A 12 step program is helpful for substance issues. Peace and good health. Sounds like a lot but once coordinated it will be helpful.
