Pap smear unrelated: A pap smear screens for cancerous/pre-cancerous cells of the cervix. It does not screen for vaginal or cervical infections or uterine/cervical/vaginal bleeding which can be the cause of a foul vaginal odor. Your clinician should perform a vaginal exam, and possibly collect cultures and/or a wet mount to help determine the cause of your foul vaginal smell.

Abnormal bacteria: Bacterial vaginosis is an overgrowth of the wrong bacteria and a deficiency of the good bacteria that defend the vagina and keep the correct ph balance. This leads to a bad odor that is typically "fishy" smelling. It is not an std, and it will not cause an abnormal pap smear. It responds to antibiotics, but often recurs. Vaginal probiotics may help.
