Congestion: As left atrial pressure increases, the pulmonary vasculature becomes congested until frank transudation occurs (pulmonary edema) if it is acute. In the chronic state, pressure in the pulmonary artery rises. The right ventricle fails. The right atrial pressure rises. There is systemic venous congestion and frank bi-ventricular failure results.
