Many reasons: It could be a that the herniation worsened between the scans. It could also be a new herniation that happened between the two scans. It could also be that it is a small herniation that the first radiologist did not think was significant and the second thought it might be because the patient was having persistent pain. Sometimes, one set of pictures just shows it more clearly than the last too.
