I've had for some years in the back of my head right side a painful hard lump that once in a while becomes swollen & causes head-ache. what can it be?

This may be: The muscle overlying the greater occipital nerve just below the skull base. If it is tender to push on you might try using a heating pad. If it continues see either a neurologist or an old laryngologist. I used a steroid injection which often relieves the symptoms.
