Same as in children: If you truly have a middle ear infection, otitis media, and the cause can be proven to be bacterial, the same bacteria that infect the middle ears in childhood, cause otitis media in adults. And therefore, the same treatment is administered. So, first, an appropriate diagnosis needs to be made and if the cause is bacterial, the correct antibiotic needs to be given for 7-10 days.

Depends: If evaluated by a dr. And a history and physical exam is performed with a visual inspection of you ears, a diagnosis can be forthcoming regarding whether it is viral or bacterial. Bacterial would require antibiotics, whereas a viral infection would require more supportive care of the symptoms (nsaids, ant-pyretics, etc.).
