Exam indicated: It is likely that you had a viral upper respiratory infection weeks ago, but with these symptoms now, a bacterial infection may be present which may require appropriate antibiotics for treatment. You should be examined to determine what type of infection you now have and whether a chest xray is indicated.

If you are a smoker: Then stop. If you have not yet had a chest xray recently then i would see your doctor for an exam and a chest xray at his/her discretion. It may be just a protracted URI but you should see a physician soon.
