For a week i have had occasional left shoulder pain, left upper arm pain, and tingling fingers at night. could these signal a heart attack coming?

Nerve: It is more likely that you have nerve entrapment referrable to a neck disk bulging. A neurologist could evaluate you for this. To look for coronary artery blockage, a maximal treadmill stress test is the best test for you. I think your symptoms are more likely from the neck rather than the heart.
