Diltiazem cream: Anal fissure often causes spasm of the internal anal sphincter with painful stools. The spasm leads to external (and internal hemorrhoids). Many get relief of the spasm and pain with topical calcium channel blocker creams such as Diltiazem or nifedipine. Low concentration Nitroglycerin cream is also effective but about 10% of patients get headaches. Also need to keep the stool soft.
