Difficult answer: May be because the chromosome analysis does not fit the genital picture, or it can be "psychological". It is possible to raise a boy as a girl and vice versa in the first 2 years of life and it sticks. Hormones are not the only answer.

Depends: Children who behave in a manner differently that what is typical for their gender may be called gender-variant. Some gender-variant children may be transgender or trans-sexual one day, but many are not. A child exhibiting gender-variant behavior may experience some distress about being a girl or a boy, in which case they may have gender identity disorder.
