Anatomic differences: In a spinal block, the local anesthetic is placed in the spinal fluid; whereas in an epidural block it goes into the epidural space or space around the spinal chord. The latter works by diffusion of the drug to the nerves whereas the former the onset is faster and more predictable. Both have advantages and disadvantages.
