Accidental: cross contamination happens if something gluten free is grown in a field next to wheat/gluten which then blows over into the gluten free field and thus, its contaminated, or, it is processed in a factory that also processes gluten products without proper cleaning or separation. This idea could easily happen in a restaurant's kitchen too.
