Money: Case management is insurers trying to save money;disease management is trying to improve outcomes days, weeks, and years down the road by applying "best , evidence-based practices" to our current treatments.It is not an easy balancing act.We have been surprised more than once in the quest for the "right" treatment for a disease process.
