Muscle spasm: Forties female with pain occiput of skull and dizziness describes chronic muscle spasm of superior trapezium muscles. A muscle relaxant or benzodiazepine might help and confirm the diagnosis. A good doctor to evaluate would be an osteopath. A neurologist can inject the superior trapezium tendons to relieve the pain. Then begins search for the cause, which might be shoulder girdle asymmetry.
