Origin of pain: A 4 year old should be able to tell you where the pain is coming from. If it is from outside ear & pain increases if you tug on the ear it is a canal issue like swimmers ear=no fever. If it is from inside it may not be infected at all. Pain is produced when the ear drum stretches. This can happen if sucked in, like in an airplane ride, or pushed out with infection. An exam is needed to sort it out
