8-12 weeks, but...: Standard answer is 8-12 weeks. But the truth is that for most people, the effects of antidepressants are indistinguishable from placebo. Approx 15% of depressed patients (give or take) have a meaningful response to the the drug; for everyone else, we are basically seeing placebo effects. Meds alone are not the treatment of choice for depression. Psychotherapy should be part of your treatment.

Reassess about now: Most people start seeing improvement in depression in about 2 wks. But it can take much longer; some say 8 or even 12 wks. Opinions differ on how long to wait. I reassess about 4 wks after starting. If no benefit by then, i either up the dose or try something else. If the tiredness is due to lexapro, (escitalopram) your doc may want to stop it anyway. Psychotherapy can help in addition or instead of meds.
