Depends what tried: Patients can be treated conservatively or surgery.Reasons for surgery: leak > 1 liter/day for 5 days or persistent leak for > 2 weeks with conservative management, nutritional or metabolic complications, including electrolyte loss, immune compromise, loculated chylothorax, fibrin clots, or trapped lung.Conservative includes fat-restricted oral diet with med. Chain triglycerides.Repeat thoracentesis.
