Fair: It is extremely unusual to have both mutations. They are mutually exclusive. Patients that do carry one of these mutations are eligible for various medications that target these mutations. Crizonitib for alk and tarceva (erlotinib) for egfr and new agent recently to the market. These new finding have improved the prognosis. Other factors do enter in. Age, ps, smoking history if any, wt. Loss, etc.

Negative pressure: You can die from collapsed lung, as it progresses quickly to a tension pneumothorax. First responders, emt and trauma providers are trained to recognize and release the pressure associated with a collapsed lung, to prevent tension pneumothorax. A chest tube is inserted into the space between your lung and chest wall. The tube is put to negative pressure, so lung can re-expand.

Possibly: In the adjuvant setting a hormone blocking agent such as an ai might be used. That would be for hormone sensitve tumors. In metastatic disease the situation can be more complicated. In that case Herceptin could be used as a maintaince agent. Other drugs such as tykerb, (lapatinib) perjetta might fill this role. If you could give more details, i can be more specific.
