Impossible to say: First, am not sure anybody is doing blood tests for nicotine or cotinine, because neither stays in blood more than a few hours. Urine testing is the gold standard, and remains positive for about 2 days after a single exposure. There is nothing healthier about using a hookah, and you should strongly consider discontinuing your tobacco habit before it harms your health, rather than fear of a test.
