Misunderstood: Yes, antisocial personality disorder is somewhat misunderstood as we are discovering that there are some subtypes of antisocial - some who have significant criminal incarcerations - some who have not. The emotional symptoms are similar among the subsets but the behaviors are different. Also, there is subset of antisocial who have deficits in the brain that can be seen in pet scans ; born with it.

Antisocial: The hallmark of this disorder is the person seems to have no conscience, or sense of morality, or right or wrong. This person cannot empathize with others, is opportunistic, predatory, and often thrill seeking. An example of this would be Charles Manson. The mystery about it is the nature and nurture aspects of what causes it. The behaviors of it on the other hand are quite clear.
