Several: Smelly vaginal discharge, pelvic pain if infection severe.

Fever: The most likely thing that will happen with a lost tampon is that you will develop a foul smelling discharge. Unfortunately, "toxic shock syndrome" is a rare but potentially life threatening disease. The symptoms may start suddenly, and you may not have a lot of time to get treatment after that. Look for fever/chills, rash, body aches, nausea, vomiting, diarrhea, and swelling.
