A million things: All of them potentially bad. Smoking irritates the bronchial passages, the vocal cords and everything in between, and something is likely inflamed. You may also have acid reflux disease, also increased by smoking. This is also potentially worsened by ibuprofen. Nobody can diagnose this from just your description. You need to see your pcp and get an examination, and may need labs/xray as well.
