Synonyms: The terms bulging disk, slipped disk, herniated disk are roughly the same idea that they disc has moved out of place where it should be and possibly compressing nerve or spinal cord. Typically though herniated disk signifies a slight increase in severity of pathology over a bulging disc. It gets a little more specific if terms like disc protrusion and disc extrusion come into the mix.

Slipped is misnomer: A slipped disc is a misnomer, but tells us it's a disc problem. A bulge is a weakening of the annular fibers causing change in shape of disc. Herniation is a shift in the nucleus of the disc. Protrusion or extrusion causes nucleus to shift outside the disc. All may involve a tear which often causes chemical irritation of nerve roots and pain. Direct nerve root contact may occur with the last 2.

The disc is like a jelly doughnut where the inner part (nucleus pulposus) squeezes out through a tear in the outer part (annulus fibrosus). Similar to torn cartilage in the knee (torn meniscus), there can be a tear in the cartilage of the disc.
