#1 Rule is don't: Cram for exam. Study in blocks of time beginning early on. #2 don't do all -nighters. #3. Don't go too heavy on that caffeine. #4.Breath in slowly ; deeply through your nostrils. Fill your lungs with air (but not to the point of discomfort). Hold for a count of five then slowly begin to exhale through an open mouth. Repeat at least 12 times, longer if you need to. Do this before ; during test.

Focus..: Personally i wouldn't suggest any drugs out of fear that trying something new during the exam may compromise performance. Do study of course, give yourself some time off immediately prior, have some good ritual with your friends ( we used to eat italian pasta the night before, for example). If you got that far, being medical student, you should already have it in you. Good luck!

Preparation: To avoid anxiety before an exam, you have to know your subject matter, be well rested and avoid stimulants like caffeine, nicotine. Or adderal -like medications. Taking tranquilizers or sedatives is not a good idea as you want to remain alert. Eat wisely before the exam and take slow deep breaths if you feel tension during the exam. Some anxiety is to be expected, however.
