Hypospadias: You're correct and likely have a distal shaft hypospadias. Urine and ejaculation always come out the same hole since they have a common channel beginning in the prostate area. Any groove or what may appear to be an opening on the head of the penis is blind ending and not connected. Most distal shaft hypospadias will not affect fertility or sexual activity. Surgery is only treatment option.

How far underneath: Double urethra or y shaped urethra as ur urinating ; ejaculating through both openings. Lower or hypospadiac hole is usually the major opening with greater urine flow than normally placed hole. Sex ; fertility should both be ok provided "abnormal hole" is located at or beyond coronal sulcus (groove user head of penis) worth consulting expert pediatric urologist (yes at age-21) it is further back.
